Valley Mental Health and Wellness does not participate in insurance plans at this time and does not process insurance claims.
Patients may choose to independently seek reimbursement from their insurance plan, Health Savings Account, Flexible Spending Account or other community resources. The practice can provide a receipt and/or a superbill you may use for reimbursement upon request.
Payment method is by cash, check, or money order.
Alternative electronic payment methods including credit cards, debit cards, and cash apps accepted with a $5.00 per transaction convenience fee applied.
As a courtesy, I have provided links to select insurance plans’ out of network health benefits claim forms. Please refer to your specific health insurance policy to ensure you have correct form for your insurance submission.

$150Individual psychotherapy, family therapy, behavior modification, education, and coaching to promote overall wellness.
$100Coordinating care and resources through advocacy, support, follow-up, and collaboration with family, support systems, and treatment providers.
$20 Per 15 minutes, or more than 10 minutes if additional action is required.Immediate, unscheduled intervention or appointment for established clients experiencing acute distress, within the limits noted in the emergency policy.
$200Formal letters, forms, care summaries, medical record requests, or other documentation to external parties outside regular business collaboration.
